MINORS:  You must have a parent or guardian present and both parties must have proper photo identification.  If the last names and addresses do not match, we will also need a birth certificate or guardianship papers for further proof.  This is a MUST and required by our Health Department.  Sorry, NO EXCEPTIONS.
